Prices are subject to change. Fees are based on the usual and customary fee at the time of service. It is the sole responsibility of the member to maximize their benefits by arranging all appropriate appointments within the 12-month membership period. If appointments are not used, the member will not be entitled to a refund.

- Our Care to Smile Savings Plan is not a dental insurance plan and can only be used at Charmoy Dental Associates.
- The plan cannot be combined with other insurance plans, discounts, specials, or coupons.
- The plan is not applicable to any treatment prior to joining.
- A 12-month membership is required, with the fee due in full upon joining.
- The plan becomes effective the day the plan payment is received.
- Payments are due when services are rendered.
- The plan is non-transferable and non-refundable.
